During the Initial registration process, candidates are required to submit these documents to secure the conditional acceptance letter and complete the admission process (for getting the conditional acceptance letter for the candidate, we need only copies of high school or degree certificates and their transcripts; so candidates can arrange the apostle and authenticated documents after getting our conditional acceptance letter. So they can do it as a second step):

1. AMA-AP completed application form

2. Short CV

3. Passport copy

4. A photo for identification

5. Personal questionnaire/survey (this item will be provided to the candidate after issuing the conditional acceptance letter).

6. Scan of the maturity certificate (with apostille), that the candidate is eligible to attend a Bachelor's degree program (it is not necessary in many cases, as long as the candidate provides high school and or bachelor's degree certificate with apostille or authenticated by the appropriate ministry by candidate’s home country).

7. Scan of the high school diploma (12th grade or 10 + 2 or A1 level diploma) and or bachelor's degree diploma (with apostille); all academic documents (diploma/degree certificates and transcripts) must be apostille and authenticated by the concerned ministry of candidates’ home country).

8. Motivational letter (candidates need to write two paragraphs about why she/he wants to be a nurse and can also explain his/her professional goal, etc.).

9. Certificate of statement of English proficiency [Regarding English language proficiency requirements, a document certifying that she/he completion of previous studies (high school or the last four years of education) in English or presenting an IELTS score of 5.5 or a Duolingo test score of at least 85 will be adequate.]

10. Required good health certificate or medical certificate by a certified medical examiner from the candidate’s home country; this certificate can be provided later, ie., after getting a conditional acceptance letter. [For Nursing, candidates must provide a medical certificate based on the referral issued by them for medical examinations, according to the Polish Regulation of the Minister of Health dated August 26, 2019, regarding medical examinations (Journal of Laws 2019, item 1651, as amended).]

Note: There are some other procedures candidates have to do and may also require to provide some additional materials for completing the admission process and for securing the Polish visa; those things can be done after submitting the initial application and getting the conditional acceptance letter from AMA-AP.
